  在内固定技术发展过程中,所采用的金属材料也从不锈钢逐步转变为钛合金。临床实践表明,钛合金具有更好的组织相容性,可以在人体内永久放置。然而,人体共有206块骨骼,形态各异,没有任何一块骨骼的形状是规则的,由此带给骨科医生的难题是,有时会因内固定器材的形状与人体骨骼的结构不相匹配,使我们面对某些部位或某些种类疾病的治疗时仍感到束手无策。

  关键时刻,3D打印技术为袁先生点亮了希望之光。为了完成这例国内外从未有人尝试过的手术,北医三院骨科组成专门团队并制定了周密的治疗方案,手术分两期做:第一期,先经后方入路将五节病变脊椎的后部结构整块切除,采用目前行之有效的椎弓根螺钉固定系统进行固定,然后打印出患者胸腰椎骨骼的解剖模型,再根据模型数据打印出替代前方五节病变椎体的钛合金人工椎体;第二期手术,经前方入路整块切除五节病变椎体,并将3D打印钛合金人工椎体安装到相邻正常椎体之间。同时再次打开后方切口,以建立后方椎弓根螺钉固定系统与前方钛合金人工椎体之间的坚强连接,从而使前、后方固定装置连为一体,确保手术后患者可以站立行走。

  2016年5月6日,第一期手术历经10个小时获得成功,术后一边等待患者身体状况恢复平稳,一边设计并打印钛合金人工椎体。一周之后,人工椎体打印出来了,它的形态和曲线完全仿照患者脊柱的解剖结构,长度达到19厘米。又过几周,患者的各项生理指标终于达到正常。6月12日,第二期手术按计划进行,历经7个多小时顺利完成,由此,跨度达五节胸腰椎的3D打印钛合金人工椎体担当起支撑人体脊梁的重任。

  两次超大型手术之后,经过医务人员的精心治疗护理,再加上袁先生积极配合康复锻炼,四周之后他便可以下地行走了。出院之后,袁先生进一步加速康复,3个月开始打太极拳,6个月开车、骑车,接送女儿上、下学,今年5月居然可以长途跋涉至南海西沙群岛。术后一年的检查结果显示,钛合金福彩3D助手人工椎体的两端已经与相邻的正常椎体融合成一体。

  3D打印钛合金植入物在医学领域应用的价值究竟如何?一条鲜活的生命得到挽救,一个幸福的家庭再次充满生机。像袁先生这样的案例远不止一个。从专业学术角度看,解决一个或一批疑难病例的治疗难题还只是3D打印金属内植物应用价值的一方面。透过病例治疗结果的观察和研究,可以看到带有微孔的金属内植物可以和其相邻的正常椎体发生很好的骨性融合,似乎预示骨融合技术以及相关理论正在发生颠覆性改变,这种技术和理论上突破的价值将难以估量!

  令人欣慰的是,在金属3D打印医学领域应用和技术创新方面,中国走在了世界前列,3D打印钛合金内植物最先撑起了中国人的脊梁!
